Kaal Sarp Dosha: What It Is and How Serious It Really Is

In short
Kaal Sarp Yoga forms when all seven planets fall between Rahu and Ketu. It has twelve types, determined by which house Rahu occupies. If even one planet sits outside the axis, the yoga is not complete.
Kaal Sarp Dosha is probably the most commercially exploited term in modern Indian astrology. It is worth separating what the condition actually is from what it has been made into.
The yoga forms when all seven classical planets, from the Sun to Saturn, fall on one side of the axis formed by Rahu and Ketu. Because Rahu and Ketu are always exactly opposite each other, they divide the chart in two. If everything is hemmed into one half, the yoga is present.
There are twelve named varieties, determined by which house Rahu occupies, each with its own traditional description. Anant, Kulik, Vasuki, Shankhpal, Padma, Mahapadma, Takshak, Karkotak, Shankhachud, Ghatak, Vishdhar and Sheshnag.
The general effect described in the texts is one of obstruction and delay: effort producing results later than expected, and progress arriving in sudden bursts rather than steadily.
Three qualifications deserve emphasis.
First, if even one planet falls outside the axis, the yoga is incomplete. What is often sold as Kaal Sarp is in fact a partial formation with a much milder effect.
Second, the condition appears in a great many ordinary and successful charts. It is not rare, and it is not a verdict.
Third, and most importantly, no chart should be read on the basis of a single feature. A strong ascendant lord, a well placed Jupiter, or a favourable dasha can outweigh it entirely.
Where remedies are wanted, the traditional ones are Naga worship on Naga Panchami, the Mahamrityunjaya mantra, and the seed mantras of Rahu and Ketu. Any practitioner who describes this condition as a catastrophe requiring an expensive ritual is worth walking away from.
